Self-employed income record template
Your income for tax is what you were paid, and when. This is a blank sheet for recording it as it arrives, one line per payment.

Date it arrived
The date the money landed decides the tax year for most sole traders. Write that date, not the invoice date.
Add the invoice number
It links the payment to the invoice, which is what you reach for when a client says they have already paid.
Total each tax year
Add the amounts for 6 April to 5 April. The total is your gross income, the number the £1,000 allowance and Making Tax Digital are tested against.
Keep the number for every month
SideTill is the simple record of your side hustle: log the job, send the invoice, record the money when it comes in, and see whether the month actually made you money. It also shows your best month, so you can tell a bad month from a bad habit.

Questions
What about cash?
Cash counts. Write it down the day you are paid.
Do I list tips and extras?
Yes, anything you were paid for the work.
Is a spreadsheet better?
A spreadsheet is fine and adds up for you. A printed sheet is for people who would rather write it down.
